Membership ID

Members are reminded that they may apply for issuance of their AMOSUP ID only up to six months from the date they have disembarked an AMOSUP covered vessel.

  1. Go to AMOSUP Records Section located at G/F SEN’IN NO IE from Monday to
    Friday 8am to 3pm
  2. Front desk staff will hand in Membership / Updating form
  3. Fill out form completely
  4. Submit the form together with the required documents (have it checked and
    validated)
  5. Wait to be called for I.D. capturing
  6. Claim your I.D.
  1. Go to AMOSUP Records Section – AMOSUP Seaman’s Hospital Cebu
  2. Member must present the requirements for applying AMOSUP I.D.
  3. In charge will then verify if the vessel is CBA approved. If approved, the member is
    given form to be filled-out
  4.  Collect and check all the requirements from the member.
  5. Member is advised to wait for I.D. capturing
  6. After I.D. capturing let the member affix his/her signature
  7. Member will sign the logbook
  8. Release the membership I.D. to member

  2. Fill out application forms (available at the Records Section)
  3. Comply and submit required documents
  4. Verification
  5. Picture taking and I.D. processing
  6. I.D. Releasing
  1. Go to AMOSUP Records Section AMOSUP Seaman’s Hospital
  2. Validation of requirements
  3. Submit complete filled- out Membership form to Records section
  4. Once the application is approved, member will proceed to JSU Building for I.D.
  5. Capturing and releasing

Note:
*AMOSUP I.D. application is accepted, processed and released on the same day. For
“Members” who wants to avail hospital benefits always bring your AMOSUP CARD and
ORIGINAL or Photo Copy of SEAMAN’S BOOK (pages w/ last vessel’s name and sign
off date stamp).

Please bring any ONE of the following requirements when applying for AMOSUP Membership Card:

  •  Seaman’s Book (page w/ last vessel’s name and sign off date stamp)
  • Latest pay slip / allotment slip
  • Updated passbook from Provident Fund
  • Certificate of Sea Service from manning agent
  • Last contract of employment
  • Endorsement Letter from manning agent

For lost or stolen Membership Card please submit the following:

  • Any ONE of the requirements above
  • And proceed to Records department front desk for replacement of lost AMOSUP
    Card

Any member who fails to provide any of the above requirements will not be issued his/her AMOSUP Membership Card.

Payment for Membership Card

  • For new application – Free
  • Replacement for lost I.D. – Php50.00
  • Retake and change rank – Php10.00
  • Active – These are members who are onboard on AMOSUP registered vessels
    with Collective Bargaining Agreement (CBA), the member will be classified as
    such within six months from his/her last disembarkation date.
  • Inactive – These are members whose date of disembarkation from a vessel
    covered by an AMOSUP CBA is more than six months.

FREQUENTLY ASKED QUESTIONS

All Seafarers who has boarded a vessel covered by an AMOSUP CBA may apply for issuance of an AMOSUP membership card.

You can apply your membership card within six months after your disembarkation date from a vessel covered by an AMOSUP CBA.

In getting your AMOSUP ID, please bring your Seaman’s Book or bring any of the following requirements:

  • Latest pay slip / allotment slip
  • Updated passbook from Provident Fund
  • Certificate of Sea Service from manning agent
  • Last contract of employment
  • Endorsement Letter from manning agent

Yes, you can also apply your AMOSUP membership card in Cebu, Iloilo and Davao.

No, you are now considered an inactive AMOSUP member.

Membership card is only issued within six months from your last disembarkation date from AMOSUP registered vessel.

YES, it is necessary to update your record every time you board an AMOSUP registered vessel. Please update your record within six months from your last disembarkation date and bring your seaman’s book or one of the requirements  listed above for verification.

YES, you may request for issuance of a Certification from our office. Please bring your Seaman’s Book for verification.

You may visit the AMOSUP records Department and request for Hospital Verification. 

note: as much as possible please bring your membership card and Seamans book if you want to avail of the hospital benefit.

As much as possible, we would like to remind you to always have a photocopy of your Seaman’s Book before surrendering it to your Manning Company since this is the best proof that you have boarded a vessel covered by an AMOSUP CBA.

If you don’t have with you your Seaman’s Book, please bring your latest pay slip showing that you have boarded a vessel covered by an AMOSUP CBA (please make sure that the name of vessel and date of issuance of pay slip are indicated). Please present the same to the hospital medical record  with your membership I.D.

None, we do not have any other hospitals available except the three hospitals mentioned. However, we have a clinic in Dagupan  Pangasinan located at DAGUPAN DOCTORS VILLAFLOR MEMORIAL HOSPITAL. 

To members who would like to avail the services of these facilities, please bring your AMOSUP membership I.D. and seaman’s book. 

While dependents who would like to avail of the services of these clinics are requested to bring their latest allotment slips and dependents card.

Yes, as long as you are an active member you may apply for issuance of a new AMOSUP membership card. However, a fee will be charged for replacement of your AMOSUP membership card

NO, you don’t need to secure an affidavit of loss since the records department has an Application form for REPLACEMENT OF LOST MEMBERSHIP AMOSUP CARD. (Please proceed to Records Department front desk) 

Sorry, but only active AMOSUP member can get a membership card. 

However, you may visit the AMOSUP records Department and request for Temporary ID number.

None, 1) new members or 2) those who has for the first time requested for issuance of an AMOSUP membership card will not be charged any fee/the member must be active. 

Temporary ID number is issued by Records department to member who wants to claim his fund in AMOSUP Provident Fund Office  and has not secured the AMOSUP membership card for the reason that the member has disembarked the vessel more than six months from his last AMOSUP registered vessel

Verification is an approved clearance from AMOSUP records department to ensure the member and the dependent is a bona fide member of the Union and to inform AMOSUP hospitals medical records department for the member’s total vessel experience covered by the Union. 

  • Who can secure the AMOSUP Verification?
    • Dependents who cannot present their allotment slip and dependents card in the hospital.
    • Members who cannot present their Seaman’s Registration Book (Seaman’s Book) and membership card in the hospital
    • Or the member has disembarked more than six (6) months from his/her last amosup registered vessel.
